Let’s understand what causes ageing skin

Many things cause our skin to age. Some things we cannot do anything about; others we can influence. One thing that we cannot change is the natural ageing process. It plays a key role. With time, we all get visible lines on our face. It is natural for our face to lose some of its youthful fullness. We notice our skin becoming thinner and drier. Our genes largely control when these changes occur. The medical term for this type of ageing is “intrinsic ageing.”

We can influence another type of ageing that affects our skin. Our environment and lifestyle choices can cause our skin to age prematurely. The medical term for this type of ageing is “extrinsic ageing.”

  • The sun – UVA and UVB rays are present 365 days per year. USA rays can penetrate through four inches of glass. So think about this when you are sitting next to a window in the office or driving in your car.
  • Smoking –  I know, we are all told how smoking is bad for your health but smoking greatly speeds up how quickly skin ages. It causes wrinkles and a dull, sallow complexion. Why, because the toxins in cigarette smoke replace the oxygen content of your blood.
  • Alcohol – Alcohol is rough on the skin. It dehydrates the skin, and in time, damages the skin. This can make us look older. So drink alcohol in moderation and for every alcoholic drink, drink two glasses of water. This will help to re-hydrate you.
  • Unhealthy diet – Findings from a few studies suggest that eating plenty of fresh fruits and vegetables may help prevent damage that leads to premature skin ageing. Findings from research studies also suggest that a diet containing lots of sugar or other refined carbohydrates can accelerate ageing.
  • Lack of exercise – Findings from a few studies suggest that moderate exercise can improve circulation and boost the immune system. This, in turn, may give the skin a more-youthful appearance.
  • Over Moisturising – Believe it or not, over moisturising the skin will send your skin cells to sleep. When your skin is slightly dry, your skin cells react by producing more collagen. If you over moisturise the skin, you will be inhibiting your own collagen production. Use a sunscreen in the morning instead! Mind you, it needs to be a UVB and UVA Sunscreen of at least factor 30 for it to really protect your skin.
  • Skin Scrubs –  Scrubbing your skin clean can irritate your skin. Irritating your skin accelerates skin ageing process. Gentle washing helps to remove pollution, makeup, and other substances without irritating your skin. Some anti-ageing products prescribed by a dermatologist may burn or sting. When using a prescription anti-ageing product, this can be okay.

Even people who already have signs of premature skin ageing can benefit from making lifestyle changes. By protecting your skin from the sun, you give it a chance to repair some of the damage. Smokers who stop often notice that their skin looks healthier. By taking some preventive actions, we can slow the effects that this type of ageing has on our skin.

There is so much conflicting advice out there

If signs of ageing skin bother you, you may ask a beauty therapist for advice. Help, my child has eczema!

Eczema, also known as ‘atopic eczema’ or ‘atopic dermatitis’, is a skin allergy/condition causing inflammation and intense irritation. When it occurs in young babies and children it can be really upsetting for whole family.

Eczema symptoms tend to be caused by dry skin. Skin that is affected by eczema gets sore and broken when it is scratched. It can look wet and may bleed. Scratching is hard to avoid since the main distressing symptom of eczema is unbearable itching, but once the skin gets broken and cracked, infections can set in, causing even more discomfort.

What Does Atopy/Atopic Mean?

There are many types of eczema and dermatitis:

  • Atopic Eczema / Atopic Dermatitis
  • Discoid Eczema
  • Contact Dermatitis

Atopy, or being atopic, means having a genetic tendency for your immune system to make increased levels of IgE antibodies to certain allergens. An atopic individual is likely to have more than one allergic condition during their lifetime, such as eczema, asthma, hay fever or food allergy.

Can Food Allergies Cause Eczema?

Children are born with the tendency to have eczema, and many things can make their eczema worse. These are known as eczema ‘triggers’. Eczema in children can have various triggers, of which food can be one, especially in babies. However, foods are not the primary cause of eczema.

If you think your child may be allergic to something and do not know what it is, you should start to keep a record of the symptoms. In particular, the following information may help us make a diagnosis

  • Do the flare ups occur at any particular time of the day?
  • Does your child only get symptoms at certain times of the year?
  • Does your child suffer more at night time or during the day?
  • Do the flare ups occur when your child is in the house as well as outside?
  • Does exposure to animals bring on the symptoms?
  • Do you think that any food or drink brings on the symptoms?
  • Do the symptoms occur every time your child comes into contact with the allergen?
  • Do the symptoms improve when you are on holiday?

Do you think I have Rosacea?

Rosacea is a common rash, found on the central part of the face, that usually occurs in the mid-thirties or early forties. It can occur in both men and women who have no history of acne as a teenager.

Rosacea is a progressive skin condition that often begins with a tendency to flush (blush). After a while this is followed by persistent redness on the cheeks, chin, forehead and nose, and by crops of small inflamed red bumps and pus spots. Rosacea can also affect the eyes, making them irritated.

What causes Rosacea? 

The cause of rosacea is not fully understood, but many think that the defect lies in the blood vessels in the skin of the face, which dilate too easily and recent studies are linking rosacea with the Demodex mite, that feeds off of our dead skin cells. Rosacea is more common in women than in men, and in those with a fair skin who flush easily but men tend to get a more aggressive form of Rosacea.

Many things seem to make rosacea worse, but probably do not cause it in the first place. They include alcohol, too much exercise, both high and low temperatures, hot spicy foods, stress, and sunlight. Things that stir up one person’s rosacea may well have no effect at all on the rosacea of someone else. The idea that rosacea is due to germs in the skin, or in the bowel, has not been proved. Rosacea is not catching.

Is Rosacea hereditary?

Rosacea does seem to run in some families but it is still not clear whether heredity plays a big part in this.

What are the symptoms of Rosacea?

Rosacea starts with a tendency to blush and flush easily. After a while, the central areas of the face become a deeper shade of red and end up staying this colour all the time. The area becomes studded with small red bumps (papules) and pus spots, which come and go in crops. Small dilated blood vessels (telangiectasia) appear, looking like thin red streaks. Scarring is seldom a problem.

Other problems with rosacea include the following:

  • Rosacea can lead to embarrassment, anxiety, or depression, and a disrupted social life. 
  • The face may swell (lymphoedema), especially around the eyes. 
  • The nose may grow big, red and bulbous (rhinophyma) due to the overgrowth of the sebaceous glands. This is more common in men than women. 
  • Some people with rosacea have eye symptoms (red, itchy, sore eyes and eyelids; a gritty feeling; sensitivity to light). A few patients with rosacea have more serious eye problems, such as rosacea keratitis, that can interfere with vision.

What causes acne?

The oil-producing (sebaceous) glands are particularly sensitive to certain hormones present in both males and females. These hormones stimulate the production of excess oil. At the same time, the dead skin cells lining the pores clog up the follicles. As a result, there is a build up of oil, producing blackheads and whiteheads. Acne bacteria lives on everyone’s skin and in those prone to acne, the build up of oil creates an ideal environment in which the bacteria can multiply. This triggers inflammation and the formation of red, pus-filled spots that can be quite uncomfortable.

Some acne can be caused by medication given for other medical conditions or by certain contraceptive pills or injections. Some tablets taken by body-builders contain hormones that can trigger acne and other problems.

How will your acne be diagnosed?

There are several varieties of acne that Sue Ibrahim will be able to diagnose at your dermatology consultation. We have extensive experience in dealing with all types of acne. We will discuss the treatment options available to you that can be very effective in preventing the formation of new spots and scarring.

How can acne be treated?

Acne treatments fall into the following categories:

  • Treatment with topical creams (prescription and/or non-prescription)
  • Treatment with oral antibiotics alongside topical creams
  • Treatment with oral hormones that counteract the hormones that can trigger acne
  • Isotretinoin tablets (Often referred to by the trade name Roaccutane (R))

1. According to Sue Ibrahim the most crucial thing you must do is simply wash your face

He says: “First and foremost if you are going out at night you need to have a good face wash. When you wash your face the most important thing is getting rid of the dead skin cells as well as the pollution.

“The product needs to suit your skin type; so if you have oily skin then make sure you have a product that is for oily skin. It is simple things like this that we’re getting wrong when buying our skin care products.”

2. Swap the Baileys for a glass of water once in a while – hard we know!

Sue says: “Little things like drinking two glasses of water for every unit of alcohol you consume to keep your skin hydrated and healthy looking is very important.”

“It also encourages the build up of collagen in your skin that gives your skin a more plump and dewy looking glow.”

3. Turn the heating down and wrap up

Surprised? Sue explains: “Over-using the central heating and blaring the heat around your home is just drying out your skin, especially if you have sensitive skin, it can make your skin sore and itchy.

“So to counter-act this we need to stop turning on the heat full blast and enjoy wrapping up warm and cozying up in blankets instead.

“However if you are going to use the central heating it is ideal to put a bowel of water in the room so the air gets a little moist as well.”

4. Sue is convinced these 3 things will keep your skin glowing

“Get plenty of sleep, always wear SPF no matter what time of the year it is and don’t smoke!” she says.

5. The number one party-proofing product?

For Lee it’s all in the regime, he says: “With skincare you can’t just use one item you need to have a proper skin regime.”

6. But how do you prevent getting bad skin?

Sue says: “Not putting too much make-up on – people don’t tend to give their skin chance to breath.

“It will also reveal wrinkles if you wear heavy make up. Try an mineral foundation instead”

7. And what’s the biggest skincare myth floating around?

According to Sue Ibrahim, it’s the following: “The biggest skin myth at the moment is that you only need to wear SPF in the summer.

“This just isn’t true. You must wear it everyday all year round.”

8. Who has the best skin in the celebrity world?

In Sue’s opinion it’s the one and only Meghan Markle, she says: “Meghan Markle – when I look at her she always appears un-sun damaged and she has a very natural tone and she is not overly worked on.

“She also wears neutral make-up and has great strong features.”

Botox, what is it and is it safe for me?

What are BOTOX ® injections?

BOTOX ® is the brand name of a toxin produced by the bacterium Clostridium botulinum. In large amounts, this toxin can cause a form of muscle paralysis known as botulism, which is usually associated with food poisoning. Even though one of the most serious complications of botulism is paralysis, scientists have discovered a way to use it to human advantage. Small, diluted (weakened) amounts can be directly injected into specific muscles, causing controlled relaxation of the muscles.

Frown lines? What frown lines?

The FDA approved such usage in the late 1980s when it was discovered that BOTOX® could stop ailments such as blepharospasm (uncontrolled blinking) and strabismus (lazy eye). Cosmetic doctors and cosmetic nurses have been using BOTOX® for years to successfully treat wrinkles and facial creases. BOTOX ® is approved for treatment of frown lines on the forehead, crow’s feet (lines around the eye), and axillary hyperhidrosis (increased sweating of the armpits). Within the past few years, new products that have similar preparations have been introduced into the UK market and have been well-received by patients.

We will refer to the toxin as BOTOX® from here on out, but please know that this includes all of the formulations that we use at Elan Medical Skin Clinic in Essex :

How does BOTOX ® work?

BOTOX ® blocks the signal from the nerve to the muscles. The injected muscle can no longer contract (tighten) as forcefully, which causes the wrinkles to relax and soften.

BOTOX ® can be used on the forehead lines, frown lines, crow’s feet, bunny lines (lines in the nose), chin (for dimpling), skin bands on the neck, and around the mouth (for smoker’s lines and down-turned corners of the mouth). Wrinkles caused by sun damage and gravity often will not respond to BOTOX®. It is important to re-emphasise that BOTOX® is NOT a facial filler (that is, it does not fill existing wrinkles) – it merely relaxes the muscles that are creating those wrinkles.

Sunburn increases risks of skin cancer

Are you aware that sunburn in childhood or adolescence can double the risk of developing melanoma, a lethal form of skin cancer in later life?

So how should you deal with sunburn if you have been burnt over the weekend? Our Dermatology Nurse Consultant, Sue Ibrahim, explains the immediate first aid steps you need to take….

Sunburn increases risks of skin cancer in later life

1. Act quickly and get out of the sun

Cover up the affected areas and stay in the shade until your sunburn has healed. Wear loose cotton clothing that allows your skin to breathe over the sunburnt areas.

2. Take over-the-counter pain relief

Analgesia or painkillers can help relieve the pain and reduce inflammation caused by sunburn. Nonsteroidal anti-inflammatory drugs such as ibuprofen are ideal and should be continued for a period of at least 48 hours if there are no contraindications. Paracetamol will help with pain but has little effect on inflammation.

3. Cool the skin

Apply a cool compress to the skin such as a towel dampened with cool water for 15 minutes, or take a cool bath or shower. Aim to keep the temperature just below lukewarm. Make sure the shower has a gentle flow of water rather than being on full power.

If blisters are starting to develop, then a bath is preferable. Do not rub your skin with a towel, but gently pat it dry when you get out.

4. Moisturise

After a bath or shower, use an unperfumed cream or lotion to soothe the skin. Repeated applications of this are necessary to reduce the appearance of peeling and this may need to be continued for several weeks.

Aloe vera or soy-containing gels or lotions can be beneficial in soothing the skin. Aloe vera not only has a cooling effect on the skin but also acts as an anti-inflammatory.

Be wary of using creams or lotions that contain petroleum, benzocaine, or lidocaine. These can either trap heat in the skin or cause local skin irritation.

5. Stay hydrated

Sunburn can encourage fluid loss through the skin. Drinking plenty of water will prevent dehydration and help your body recover. Alcohol should ideally be avoided during this time as it will make dehydration worse.

6. Leave blisters alone

Try not to pop blisters as this can lead to infection and scarring. They will settle by themselves after a few days. In the meantime, treat the skin gently.

Remember sunburn can cause long-term damage to the skin and prevention should be the key.

Age Spots and what to do about them

Age spots, often known as liver spots, are an all too visible reminder of the passing of time. They appear in people over 40 although younger people can be affected as well. If you have noticed several flat brown or black spots on your face, hands, arms and shoulders then these are likely to be age spots.

Causes of age spots

The ageing process is the main reason hence the name. But they also occur from years of exposure to direct sunlight, often as a result of sunbathing or heavy use of sunbeds. Constant exposure to ultraviolet (UV) rays changes the colour of skin, turning it to a light or dark brown colour.

This is fine when you are young but as you get older your skin is less effective at regeneration as a result of sun exposure which leads to the formation of age spots. Plus your skin is thinner and more fragile as you age which increases this risk.

Genetics also plays a part. If your parents or a close family member has age spots then you will have inherited this condition.

Apart from genetic factors; if you have fair skin or are a keen sun worshipper then you are more likely to develop this condition. But statistically, anyone can get age spots.

Age spots do not determine your age: what they are is the consequences of years of exposure to the sun, i.e. tanning. So think carefully the next time you decide to bag some rays whilst on holiday.

Symptoms of age spots

These spots are flat, oval shaped objects which are brown, black or even grey in colour. They vary in size and develop in areas which are most exposed to the sun, e.g. hands. They develop on the arms, shoulders, back, chest and neck as single spots or in groups. They are diagnosed during a dermatology consultation using an instrument called a dermatoscope. There are other skin conditions which display symptoms that are very similar to age spots. These include seborrheic keratosis (benign skin growths) and moles. But an examination will rule these out.

Treatment for age spots

These are often an inevitable part of growing older but if you find these unsightly to live with then there are remedies for dealing with these spots. One of the most revolutionary ways of removing these spots is by a dermatology procedure called Plasma Soft Surgery

Can you prevent age spots?

If there is a genetic tendency then no but many cases are caused by prolonged exposure to the sun. So reduce your exposure and use a suncream with a high sun protection factor (SPF). Wear a hat and loose, long sleeved tops and trousers to protect your skin from the sun.

Think pale and interesting rather than bronzed and beautiful.
